John Hoh, M.D. | Medical Director
Dr. John Hoh is APHCV’s Medical Director and has been with APHCV since 1996. He specializes in Internal Medicine with added formal training in Geriatric Medicine. He received his medical degree in 1983 from Temple University. He completed a 3-year medical residency at Montefiore Medical Center and a 3-year fellowship at John Hopkins Medical Center and the National Institute of Health-Gerontology Research Center. Dr. Hoh treats patients in English and Cantonese.
Lena Al-Sarraf, D.O.
Board certified in general practice medicine, Dr. Al-Sarraf completed her medical degree at Western University and completed additional post-doctoral training at USC. She is fluent in English and Arabic. She can also discuss medical issues in Spanish. She enjoys reading, biking, sports, and music. Dr. Al-Sarraf sees patients at Belmont Health Center.
James Bon, NP
James Bon, NP, joins us with 15 years of experience in clinical roles. He previously served at APHCV for 4 years from 2005 to 2009. He returned to the Philippines to obtain two doctorates and to further his clinical experience. He has provided primary care at the Men’s Health Foundation, including addressing the needs of the LGBTQ community. He also provided preventative care and managed treatment of chronic conditions. James values being able to once again provide wellness visits and broader medical care for APHCV’s diverse populations. James graduated with a BSN from Cal State University, Dominguez Hills, and obtained his MSN and NP from UCLA. He is fluent in English and Tagalog. James provides care for our patients at Los Feliz Health Center and Belmont Health Center.
Wei Min Chan, PA-C
Min is a Florida native who attended Nova Southeastern University where she received her Master of Science in Health Sciences. She is fluent in both English and Cantonese. She enjoys exploring her new California home with her dog and trying new foods. Min sees patients at Los Feliz Health Center.
Di Chung, D.O.
Dr. Di Chung graduated from Edward Via College of Osteopathic Medicine and completed her Family Medicine residency at PIH Health. She participated in medical missions abroad, and before becoming a doctor, she helped provide care for patients experiencing homelessness and those diagnosed with cancer. She speaks conversational Spanish and Vietnamese, and is fluent in Cantonese. Dr. Di Chung sees patients at Belmont Health Center.
Jolie Cooperman, M.D.
Dr. Cooperman provides services at APHCV through our partnership with Vituity. She received her Doctor of Medicine from the Keck School of Medicine and completed her residency at the University of California Los Angeles. She also completed a fellowship program at Rochester General Hospital in Rochester, NY. She is board certified by the American Board of Family Medicine and specializes in Family Medicine and Obstetrics and Gynecology. She sees patients at Los Feliz Health Center and John Marshall High School Health Center.
Jing Deng, M.D.
Dr. Deng is bilingual in Mandarin and completed her medical education at the University of Illinois at Chicago and Bengbu Medical College in China. Her specialty is Internal Medicine and completed her residency at the Wright Center for Graduate Medical Education in Pennsylvania. Dr. Deng enjoys cooking, reading, traveling, and spending time with family. Dr. Deng sees patients at El Monte Health Center.
Shaozheng (Jennifer) Ding, M.D
Dr. Shaozheng (Jennifer) Ding is a Family Medicine Physician with extensive experience. She completed two residencies, one in China and another in South Carolina at Greenville Hospital System. She also participated in a Dermatology fellowship program at Beijing Union Medical College. She has experience with occupational injuries, outpatient minor surgery, and chronic disease management. She is fluent in Mandarin Chinese. Dr. Ding sees patients at Los Feliz Health Center.
Elizabeth Fisseha, M.D.
Dr. Fisseha received her Doctor of Medicine from University of California Los Angeles, David Geffen School of Medicine. She completed her psychiatry residency at the University of California Los Angeles, Semel Institute for Neuroscience and Human Behavior. She has a Master of Science in Global Medicine from the University of Southern California and a Bachelor of Arts in Neuroscience from Amherst College, Massachusetts. Dr. Fisseha sees patients at Los Feliz Health Center.
Raymond Goei, D.O.
Dr. Goei received his Doctor of Osteopathic Medicine degree from the Western University of Health Sciences/College of Osteopathic Medicine of the Pacific. He is certified by the American Board of Family Medicine and completed his residency at San Bernardino County Medical Center. Dr. Goei enjoys working with computers and web-surfing, home improvement, gardening, and skiing. Dr. Goei sees patients at El Monte Health Center.
Robert Howard, LCSW
Robert Howard grew up in Oklahoma and attended Boston University where he earned his Master of Social Work degree. Robert previously taught at Boston Children’s Hospital. He also has experience in working with children with various healthcare needs. Robert’s innovative work helps his patients work toward wellness. In his leisure time, Robert enjoys outdoor activities, music, concerts, and spending time with family. Robert sees patients at Los Feliz Health Center.
Kevin Ko, M.D.
Dr. Ko joined APHCV after seven years of service in outpatient and hospitalist care. Dr. Ko’s education includes Family Medicine Residency at the University of Texas Southwestern (2014) with work at St. Paul University Hospital and Parkland Memorial Hospital, Dallas, Texas. Dr. Ko enjoys traveling, working in orphanage care in Uganda and Haiti, and community development in Peru. He is fluent in Mandarin and also some Medical Spanish. Dr. Ko sees patients at Los Feliz Health Center.
Dominic Koh, MSN, CPNP, ANP-C
Nurse Practitioner and APHCV’s Director of Nursing, Dominic Koh has been with APHCV since 1997. He completed his nursing degree and a post master’s certificate at Cal State University, Los Angeles. He is a Certified Pediatric Nurse Practitioner (CPNP) and an Adult Nurse Practitioner – Certified (ANP-C). Dominic is fluent in English, Mandarin, and Cantonese, and sees patients at Los Feliz Health Center.
Laura Lai, M.D.
Dr. Lai completed her doctorate degree at the University of Southern California, Keck School of Medicine in Los Angeles, California. She completed her residency and fellowship at the LAC-USC Medical Center in psychopharmacology and psychiatry. She is a certified member of the American Board of Psychiatry and Neurology. Dr. Lai is fluent in English and Chinese. She provides care for our patients at the Los Feliz Health Center.
Perpetua Magpuri, R.D.
Perpetua Magpuri is our in-house Registered Dietician. She received her M.A. in Nutrition and Diabetes from New York University and has experience as a lifestyle coach and nutrition educator. Perpetua is excited to help patients achieve their weight loss and lifestyle goals. She provides care at Los Feliz health Center.
Wendy Mak, D.O.
Dr. Mak received her Doctor of Osteopathic Medicine degree from the Western University of Health Sciences in 2015. She completed her residency program at the Dignity Health Northridge Family Medicine in 2018 and is certified by the American Board of Family Medicine. She is comfortable providing care for newborn patients and seniors. Dr. Mak speaks English, Mandarin, Cantonese, and Spanish. Dr. Mak sees patients at Los Feliz Health Center.
My An Nguyen, M.D.
Dr. Nguyen a pediatrician who joined APHCV in 2024. She is a graduate of Wayne State University School of Medicine and completed her residency at Harbor UCLA. Dr. Nguyen is a Fellow of the American Academy of Pediatrics. She is fluent in English and Vietnamese and sees patients at Los Feliz Health Center.
Tiffany Prachachalerm, D.O.
Dr. Prachachalerm joined APHCV in 2021. She completed her medical degree in 2018 at Touro University of Nevada, College of Osteopathic Medicine. Dr. Prachachalerm is excited to help her patients achieve their health goals. In her free time, some of her hobbies include trying different cultural cuisines, cooking, traveling, and playing golf. Dr. Prachachalerm is fluent in English, Thai, and some Medical Spanish. She sees patients at Los Feliz Health Center.
Jennifer Junko Reyes, LCSW
Jeniffer Reyes, LCSW, join us with over 10 years of experience in community mental health and education, counseling, and therapy settings. She graduated from UC Berkeley with Highest Honors in Social Welfare. She has experience working on a team to improve sanitation and health care access for indigenous populations living in the Pastaza Region of Ecuador. Jeniffer has experience serving diverse populations through Community Mental Health and Student Mental Health programs, providing superb care for adults, adolescents, and children. Jenniffer provides care for our patients at the Belmont Health Center and El Monte/ Rosemead Health Center.
Alexandria Stern, D.P.M.
Dr. Alexandria Stern completed a Doctorate of Podiatric Medicine at Western University of Health Sciences, Pomona, California. She completed her medical residency at Scripps Mercy Hospital, San Diego, California and has Board Certification from the American Board of Podiatric Medicine. Dr. Stern has two young boys and enjoys hiking in her free time. Dr. Stern sees patients at Los Feliz Health Center and El Monte/Rosemead Health Center.
Kai En Tang, D.O.
Dr. Tang completed a Doctor of Osteopathic Medicine at Midwestern University Arizona College of Osteopathic Medicine. She is trained in pediatrics, geriatrics, obstetrics, adult medicine, and in comprehensive care for medical and mental health. Dr. Tang is fluent in Mandarin, Spanish, and conversational Japanese. Dr. Tang sees patients at Los Feliz Health Center.
Martin Tolosa, PA-C
Martin completed the Primary Care Physician Assistant Program at the Keck School of Medicine at University of Southern California and earned his Master of Physician Assistant Practice (MPAP) degree in 2014. He is excited to build relationships with his patients and helping them pursue and achieve their health goals. He speaks English and Medical Spanish. Martin enjoys spending time with family, running, and playing basketball. Martin sees patients at Los Feliz Health Center.
Elizabeth Wilkinson, MSN, FNP-BC, RNC-OB, RN
Elizabeth obtained her Master of Science in Nursing from California State University, Dominguez Hills and is a Board-Certified Family Nurse Practitioner. She also attended California Baptist University for her Registered Nurse degree and studied at University of California, Irvine as an undergraduate. Outside of work, Elizabeth competes in synchronized swimming and loves to garden. Elizabeth sees patients at John Marshall High School Health Center.
Phyllis Woo, MPH, WHNP-BC
Phyllis Woo brings a wealth of knowledge and care to APHCV’s women’s health program as a Certified Women’s Health Nurse Practitioner. She received her Bachelor of Science and a Master of Science in Nursing from Columbia University and a Master of Public Health from the University of Southern California. When she is not caring for APHCV patients, Phyllis can be found outdoors hiking or camping and exploring the many museums L.A. has to offer. Phyllis practices at Los Feliz Health Center.
Jack Yu, M.D., MS Ed.
Dr. Yu provides services at APHCV through our partnership with Vituity. He received his Doctor of Medicine degree from Boston University and completed his medical residency in Family Medicine at the University of California Los Angeles Hospitals and Clinics. He completed a Master of Science in Education fellowship with the University of Southern California Medical Education Faculty Development. He is certified by the American Board of Family Practice and American Board of Family Medicine. Dr. Yu sees patients at Los Feliz Health Center.
