APHCV has received three American Heart Association outpatient program achievement awards in recognition for its commitment to reducing the risk of heart disease and stroke by improving high cholesterol, Type 2 diabetes, and blood pressure management. The awards recognize a commitment to following the latest evidence- and science-based care guidelines.

Nearly half of all adults in the U.S. have some form of cardiovascular disease, including heart attack, stroke or heart failure.[1] High cholesterol, Type 2 diabetes, and uncontrolled high blood pressure are leading risk factors for cardiovascular disease, but with timely diagnosis, research-based treatment and education, these conditions can be managed.

The outpatient achievement award programs put the unparalleled expertise of the American Heart Association to work for hospitals nationwide, helping ensure the care provided to patients is aligned with the latest evidence- and research-based guidelines. As a participant in the Target: BP, Check. Change. Control. Cholesterol., Target: Type 2 Diabetes programs, APHCV was recognized after demonstrating how their organization has committed to improving risk factor management for patients.

This year, APHCV received these achievement awards:

  • Target: BP™ Gold+ recognition jointly presented by the American Heart Association and the American Medical Association.

APHCV recognized by L.A. Care’s Honor Roll

Asian Pacific Health Care Venture, Inc. (APHCV) has been recognized by L.A. Care for its first Honor Roll in which 20% of Providers in L.A. Care’s Pay-for-Performance (P4P) program are recognized for outstanding service and patient care.  We have been honored by L.A. Care for being a top performer on important clinical quality measures including delivery of cancer screenings, chronic disease management, women’s health, pediatric care, well child visits, immunizations, and more.

L.A. Care Health Plan’s mission is to provide access to quality health care for Los Angeles County’s vulnerable and low-income communities and residents and to support the safety net required to achieve that purpose.

Kaiser Permanente funds APHCV for Population Health Management Initiative

Kaiser Permanente has awarded Asian Pacific Health Care Venture, Inc. (APHCV) with funding under its Population Health Management Initiative.  The Initiative is a collaboration between Kaiser Permanente, community health center, Regional Association of California (RAC), California Primary Care Associate (CPCA), and Department of Health Care Services (DHCS) to transform care for Medi-Cal beneficiaries.  Together, these collaborative partners are working to advance population health management capabilities in order to eliminate health disparities and improve the health of people and communities. The partners focus on shared priority measures and populations including children, pregnant people, people with behavioral health conditions, and adults living with chronic conditions and preventative needs, engage resources and expertise, and invest in technology solutions. Kaiser Permanente (KP) awarded a one-year grant fund to APHCV in December 2021, and a second grant award in March 2021.  The current project will continue through December 2024. APHCV values its partnership with Kaiser Permanente and PHMI collaborative partners to work together to improve the health of our patients.

APHCV Receives Recognition from HRSA for 5th Consecutive Year

November 28, 2023 – Asian Pacific Health Care Venture, Inc. (APHCV) was among the 8 community health centers in Los Angeles County, and 15 in the state, who received Health Center Quality Leader Gold recognition along with other Community Health Quality Recognition badges from the Health Resources and Services Administration (HRSA), an agency of the U.S. Department of Health and Human Services (HHS).

The digital badges are part of HRSA’s Community Health Quality Recognition badge program which honors a select number of health centers across the U.S. that achieve outstanding performance in a variety of clinical areas including behavioral health, maternal health, diabetes health, heart health, cancer screening, and HIV prevention and care. The program also honors health centers in the top 10%, 20%, and 30% that achieve best overall clinical quality performance.

This past month, APHCV received three recognition badges for 2023: Health Center Quality Leader – Gold, Access Enhancer, and Health Disparities Reducer. APHCV has received Health Center Quality Leader Gold recognition—placing in the top 10% of health centers nationwide—for five consecutive years, and this year marks the third year it has received Access Enhancer recognition and Health Disparities Reducer recognition.

Community Health Centers are federally qualified health centers or look-alikes that receive government funding to provide medical care and services to rural or underserved communities. HRSA Administrator Carole Johnson stated, “We at HRSA are proud to support and partner with these health centers, which not only provide care to patients regardless of their ability to pay but also consistently deliver excellent care that exceeds national quality standards.”

Meet our new Podiatrist!

Dr. Alexandria Stern received her Doctorate of Podiatric Medicine (D.P.M.) from Western University of Health Sciences in Pomona, California, and completed her residency at Scripps Mercy Hospital in San Diego, California. As a podiatrist, she focuses on prevention, diagnosis, and treatment of the foot, ankle, and related structures.

Dr. Stern sees patients of all ages at Los Feliz Health Center on Mondays, Tuesdays (second, third, and fourth), Thursdays, and third Saturdays from 8:30AM-5:00PM. She also sees patients at El Monte/Rosemead Health Center on first Tuesdays from 9:00AM-5:30PM. Belmont Health Center patients are welcome to use these services, considering Los Feliz Health Center is only a 15 minute drive from Belmont Health Center.
